Please print and complete these forms and return to the school office by the first day of school (August 23).  
All forms are PDF's and can be opened with Adobe Reader.

The following forms are mandatory for all students:

2. Insurance Statement
3. Medical Authorization
4. Photo Release
5. Acceptable Use for Internet
6. Academic Integrity
7. Alcohol /Drug screening

Please return these forms with the needed information and signatures.

3. Seizure
4. Diabetes
5. Parking permit ( include $25 fee)
