Student Athletic Forms & Handbooks
Dear Bishop Parents,
The Bishop Athletic Department has partnered with FinalForms, an online forms and data management service geared toward high school athletics. Parents/students will no longer turn in paper forms/hard copies for athletics. All athletic forms including those that need to be signed annually will be filled out and signed digitally with the exception of the sports physical which will be printed off the Bishop Final Forms Portal and brought to your doctor to fill out. All other information will be saved from season to season and year to year. Finalforms also pre-populates information wherever possible for each student saving you time. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any NCHSAA update. Please be sure to print the BMHS physical and not use the one at your doctor's office so that you are sure to have all pages. Directions for printing the physical packet and uploading it are on your portal in the menu on the left marked, "NCHSAA PPE Physical for Physicians." Simply click on that tab. You will then upload the physical to the Finalforms site either via a scan or by taking a picture and uploading the picture. If uploading a picture of the physical, please make sure all pages are readable.
When you first go to our FinalForms link (listed below as "Access BMHS Athletic Participation Forms"), you will need to set up an account which is very easy. There are directions also listed below as "Parent Directions for Finalforms Online Athletic Forms/Data Management." If you require any support during the process, scroll to the page bottom and click, "user support." When you start filling out the information, you will see that there is a lot of medical information and contact information that you will fill out before you get to the forms. Some of this information may be the same information you have filled out on previous Bishop School Forms, but doing so on our Finalforms site will allow your student's coaches and athletic trainer to be able to have access to your student's medical information via Ecard. This way, if he or she is injured off site, the coach and trainer can pull up all medical and contact information for your child to give to the medical professionals providing treatment. The good news is that you only need to fill it out one time for the duration of your student's time at Bishop McGuinness. You can login and change/edit your information any time.
With regarding to knowing when your uploaded physical will expire, Finalforms has a countdown clock, so you will receive automated email reminders beginning 60 days out from the time your child's physical expires.
Information on PE uniforms, link to purchase Villainswear, information on sports fee payment, and Villains Club Membership and Banner Sponsorship will also now be listed in Finalforms in addition to our school website at www.bmhs.us. Signing this page on Finalforms is simply acknowledgement that you read that page.
We realize this will take a few minutes extra the first time you fill everything out, but we feel this will be much easier for our parent community for the remainder of your student(s) time at Bishop. Also, our coaches and trainer having the ability to provide emergency medical providers with your student's information on site will be a great resource as well.
